If this is still a problem, I have a couple of concerns/suggestions.
First, how are you attempting to use a Virtual Document activity to allow users to create documents in Notes? An activity of this type specifically connects Notes Forms to an external database. We then monitor those forms for the changes you tell it to, then push those changes to the back-end.
It sounds like that's what your other two activities that work fine are doing! That is also what we expect a Virtual Document activity to do. I'm concerned that you might be trying something unsupported, or perhaps misunderstanding some of the activity options.
Are there more pieces to this than just the VD activity?
If you were running a "normal" virtual document activity and receiving this error, I would suspect a problem with your external table. The name being illegally re-used is one known issue. You might have something defined incorrectly, and need to rebuild the external key table.
