This can often happen when two DECS activities operate against the same Notes database. They can conflict with each other, resulting in this error message.
Do you have more than one DECS activity, and do two or more operate against the same Notes database? If so, avoid doing this if you can. Otherwise, avoiding use of the Form Override option helps, as does using the option "Leave all real-time fields in document".
Finally, the information provided says that you are on release 6. If that's true, I would recommend an upgrade. Not only is 6 no longer supported, but there has been at least one known cause of this issue that was fixed in later releases.
